Does anyone know a book which surveys different forms and expressions of worship? What I mean is 'these people had experience A1 of God, which they losely base on Bible verses A2 and explain with theolgy A3', and 'these people had experience B1 of God, which they losely base on Bible verses B2 and explain with theology B3'. etc etc. Interested to understand where people are coming from broadly and see how God has been moving in different expressions in the last 60 years or so (charismatic, wimber/vineyard, pentecostal / hillsongs, reformed, Soul Survivor etc. etc). |

I think you're looking for "Exploring the worship spectrum" edited by Engle Basden - 6 contributors from liturgical, blended, contemporary, charismatic and alternative streams do exactly what you're talking about, then the others respond. Very interesting read even if it possibly raises more questions than it answers.
hope that helps,

Let me endorse Matt's suggestion. I like the dialogue format of Basden's book, allowing folks from different perspectives to interact with one another's views.
In addition, check out a couple of books edited by DA Carson.
First, Worship by the Book has an excellent first chapter by Carson on a theology of worship before exploring three different church traditions: Mark Ashton writes from and Anglican perspective; R Kent Hughes is covers free church worship; and Tim Keller explores his experiences of planting a church based on reformed principles in the heart of Manhatten. Although it's maybe not as broad in scope as what you're looking for, the depth of material (especially from Carson and Kelller) is admirable.
The other book is Worship: Adoration and Action. Three introductory essays on the theology of worship give way to essays from nine different contributors on their theology and practice of worship, covering the full spectrum of theological views as well as churches from the developed and developing world.

Another good book that might suit what you're looking for, while it might take you farther back than just 60 years, is "Jubilate II - Church Music in Worship and Renewal" by Donald P. Hustad. I've used it as a source for many of my classes and recommend it.
